We want to make it easier for you to connect with the church office. We've archived downloadable/editable (fill in the blank) forms for your use. Below you will find our CERF and Purchase Order (reimbursement) forms.

Please complete the Calendar Event Request Form (CERF) below to start the process of requesting a calendar addition. Please note that completion of the CERF does not guarantee a calendar addition. All CERFs must be previewed by MEBC staff.  If you have questions, please contact the church office and speak to Teresa Fry.

Please download and complete the Purchase Order form to be reimbursed for expenses you may have incurred for ministry at Metro East. Please secure permission for making purchases from the appropriate ministry staff member prior to making a purchase. All reimbursements must include this form and applicable receipts. We cannot reimburse without receipts. All reimbursements are subject to approval. Thank you for helping us to keep things in order and to be good stewards of what God has entrusted us with.
